Purchase Terms, Cancelations, and Refund Policy

TERMS AND POLICIES UPON PURCHASE

Upon purchase on the website, I agree to the following purchase terms and policies as well as, others listed on the website. 

CONSENT OF TREATMENT OF ADULT AND MINOR

Upon purchase, I provide consent for nutrition counseling. I also provide consent to treat my child, a minor if applicable. I am aware that I am responsible for notifying my or my child’s medical doctor before starting any nutritional program. I acknowledge this program does not replace care from my or my child’s current medical and/or healthcare practitioners. This program is based on nutritional testing, results, and educational and professional recommendation, such as but not limited to, dietary changes and supplementations, and should be discussed with my medical doctor. I understand this program does not treat any health conditions.

WAIVE THE RIGHT TO A CONSULTATION AND/OR FOLLOW-UP VISIT

If applicable, I understand if I purchase a product or service on the website without a completed professional consultation and/or follow-up visit,  I am waiving my rights to have Dr. Sulsenti fully review my health concern, medical history, and any previous lab reports so she can make a professional recommendation as to the best nutritional path for my needs. This may or may not include the purchasing of labs, supplements, and/or nutrition counseling or program. I take full responsibility for the labs/purchase and accept the consequences if any, of doing so. 

LAB ANALYSIS WITH ONE IN-OFFICE OR TELEMEDICINE LAB REVIEW VISIT

Lab purchases include a complete lab analysis by Dr. Suslenti on her own time, followed by a thorough one-lab review visit in the office or via telemedicine with Dr. Sulsenti. I understand at this time, I will be provided with lab result interpretations and an individualized nutrition management plan with professional recommendations. This may include diet changes, supplementation, and protocols specific to results. I also understand that her interpretations may be limited if I have waived the consultation and/or follow-up visit.  I also understand I will be notified how much time will be needed for this visit as this is dependent on how in-depth the results are/are not. I accept that Dr. Sulsenti does not provide multiple one-on-one reviews for multiple tests I may purchase separately or in a bundle and that all lab tests will be evaluated together in her one review visit for optimal results.

CANCELATION POLICY

There is a $50 cancellation and No-Show Fee for all functional medicine and nutrition visits with Dr. Sulsenti. We take your health and time seriously, putting aside time dedicated to you and no other patients. Dr. Sulsenti prepares ahead for your visit(s) and is committed to the allotted time allowed just for you. If a visit is canceled or missed, it cannot be rescheduled without the Cancelation and No-Show Fee paid in full.

REFUND POLICIES, PAYMENT AGREEMENTS, AND CANCELATIONS OF  PURCHASE

I understand there are no full refunds. I understand my purchase includes fees such as but are not limited to, patient email/office support, lab processing, lab scheduling, patient processing, lab visits on-premises and via telemedicine, visit preparations, menu preps, lab report processing and preparation, nutrition counseling visits, all nutrition support in and outside nutrition visits (questions, phone calls, texting, emails, etc.), outlines, menus, recipes, nutrition management plans, lab reports, etc., by Dr. Lisa and Staff. I understand I begin nutrition services with Dr. Lisa and Staff upon purchase of a lab service on this website. 

If I choose to cancel my purchase, I must notify Dr. Lisa and her staff in writing of this decision with reasons why. I understand there is a $200 non-refundable nutrition scheduling, preparing for sessions, and lab processing fee. I understand that all services rendered including, but not limited to those mentioned above, are not refundable nor are any supplements or physical products purchased. There may be an additional cancellation fee to Vibrant America with lab cancellations as well as credit card processing fees. Dr. Sulsenti and Staff will do their best to reimburse any labs canceled and orders, but it will not be in full due to the above fees and explanations. 

COPYRIGHT LAWS 

I acknowledge I am responsible for how I use Dr. Lisa Sulsenti’s nutritional information, such as but not limited to, lab forms, lab kits, lab testing and results, leaky gut testing, all food menus, recipes, nutrition management plans, and supplement recommendations. I will not reproduce or administer any information, such as but not limited to, lab forms, lab results, diets, menus, recipes, handouts (printed or digital files) or professional management plans or supplement recommendations to others for personal or business means. If I do share a recipe, I will give credit to Dr. Lisa Sulsenti at all times. I am aware this program is the sole property of Dr. Lisa Sulsenti and breaking rules may be infringing upon copyright laws punishable under the law. 

PRIVACY, TRANSMISSION, AND RELEASE OF PATIENT INFORMATION POLICIES

I agree to read and understand my rights under the Health Insurance Portability and Accountability (HIPPA) privacy laws that will be provided. I also give Dr. Lisa Sulsenti permission to transmit my nutritional and medical information as but not limited to, consultations, health discussions, lab results, history, menus, nutrition management reports, and recommendations via email, telemedicine, phone calls, fax, text, etc. I will provide her with emails, numbers, and handles where she can send my information, informing her 48 hours in advance of any changes to this agreement in writing.
